| This agreement serves as my consent to authorize Southeastern Ohio Credit Union to provide my monthly statement electronically through Southeastern Ohio Credit Union’s Web Access instead of via the U.S. Postal Service. By agreeing to receive my monthly activity statement electronically, I understand:
- I have the option to request and receive a paper copy of my monthly activity statement at any time by contacting Southeastern Ohio Credit Union.
- I have the right to withdraw this consent at any time, without penalty.
- Should I wish to withdraw my consent and begin receiving my statements via U.S. mail, I will notify Southeastern Ohio Credit Union in writing of said withdrawal of consent, at least ten
business days before the end of the effective month.
- I must be a user of Southeastern Ohio Credit Union’s Web Access and have Adobe Acrobat Reader software (version 8.0 or higher) for viewing and printing statements.
- I will be notified at the below stated email address within the first 5 days of the month when my e-statement is available. I will notify the Credit Union of any change in my email address. If the Credit Union has the email returned for bad address, my e-statement will revert back to statement delivery by the U.S. Postal Service.
